Medical Leave

What Is It?

This service provides support for Family and Medical Leave (FMLA), Faculty Care Leave, and Sick Leave Bank processes. Employees can submit leave requests, ask questions about eligibility and documentation, notify HR of a potential medical leave event, and upload required forms or medical certifications.

ARP 8.45 - – LOA – Family and Medical provides eligible employees the ability to request up to 12 work weeks (up to 26 work weeks for Military Caregiver) of job protected leave for pregnancy, childbirth, the employee’s illness or the illness of the employee’s family member. Upon receipt of a request for Family & Medical Leave, Benefit Services will verify the employee's eligibility for FMLA leave and send a letter to the employee requesting the appropriate documentation to support the request for FMLA, along with the employee's rights and responsibilities under FMLA. Notification of the request and applicable designation will be sent to the department supervisor and the unit HR Liaison.

How Do I Get Started?

Click the  Request Service button above to submit your request.

Request FMLA for yourself or on behalf of an employee: Requests should be submitted at least 30 days prior to the anticipated leave whenever possible. If the leave is unforeseen, submit as soon as practical.

Notify HR of a potential FMLA event: If you become aware of a possible qualifying event, even if the employee has not yet requested leave, submit a request so Benefit Services can provide the employee with their FMLA rights and responsibilities. This notification must be submitted within 5 days of becoming aware of the potential need.

Submit FMLA paperwork such as certification forms, release-to-return documentation, doctor’s notes, or intermittent leave reports.​​​​​​​
