Personnel File Requests

What Is It?

This service allows current and former employees to request access to their official personnel file or authorize a third party to review it. Access is provided in accordance with university policy and applicable legal requirements.

Personnel files may include employment-related documents such as an original resume or curriculum vitae, employment application, hiring forms, offer letters, signed notices of appointment and reappointment, Personnel Action Forms (PAFs), performance review and evaluation forms, conflict of interest forms, supplemental compensation forms or documents, direct deposit authorization forms or other signed ACH transaction forms, written reprimands, notices of suspension or termination, and position reclassification memo.

Who Is Eligible to Use It?

  • Current and former employees
  • Employee-authorized representatives (with written authorization and notarized approval)
  • Authorized university officials (e.g., HR Services, Legal Counsel, Office of Institutional Equity)
  • Requests made in response to lawfully issued subpoenas or judicial summons
How Do I Get Started?

Click the  Request Service button above to submit a ticket. 

After submission:

  1. Complete the Personnel File Review Request Form.
  2. Submit the form to the HR Service Center via delivery to Hadley Hall, Room 17, by submitting this service request, or by fax to 575-646-2806.
  3. HRS will send a secure link to the provided email address or service requester. Access to the personnel file will be available for three (3) days.
How Much Does It Cost?

There is no cost to request or review your personnel file.
