What Is It?
Formstack is a management tool for creating web forms. Through Formstack, you can create your own web forms to collect data from visitors to your website and members of the general public. Data collected through web forms can include survey and reservation data as well as general feedback. Forms can be embedded on public-facing websites or hosted in the Formstack platform and shared with a unique URL.
Who Is Eligible to Use It?
Academic and administrative departments at NMSU are eligible to request the creation of Formstack accounts, which can be used to create web forms for departmental use.
How Do I Get Started?
Click the "Request Public Facing Form" button above to request the creation of a Formstack account and begin creating web forms for individual or departmental use. You will be prompted to enter your name, department, billing information, and information required for the creation of your Formstack account, including the number of accounts you are requesting.
How Much Does It Cost?
Formstack accounts will be charged $50/yr for each user.
