What Is It?
Formstack is an online tool for creating and managing web forms. It allows users to collect data from website visitors or the general public, such as survey responses, reservations, and feedback. Forms can be embedded on public websites or hosted on the Formstack platform and shared via a unique URL.
Who Is Eligible to Use It?
Academic and administrative departments at NMSU are eligible to request the creation of Formstack accounts for developing departmental web forms.
How Do I Get Started?
Click the Request Public Facing Form button above to request the creation of a Formstack account
You will be prompted to enter your name, department, billing information, and information required for the creation of your Formstack account, including the number of accounts you are requesting.
How Much Does It Cost?
Formstack accounts will be charged $50/yr for each user.
